The Washington State Department of Health has taken disciplinary actions against various health care providers as of December 15, 2025. Among those disciplined, Candice A. Haase had her home care aide application denied after allegedly accepting money from a client. Ryan Robert Sprauer faced charges for unprofessional conduct due to erratic behavior and drug-related issues, while James Kamau Kirumba was charged for physically assaulting a patient. Other actions included the termination of probation for Cassandra Nichole Lembcke and Cristian Alexis Guzman, as well as an agreed order reinstating Amanda Jean Pelot’s credentials.

The department also issued cease-and-desist orders to individuals practicing massage therapy without credentials. Notable cases involved Colleta Nambuzi, who allegedly submitted false training certifications, and Tierra Lynn Talmadge, charged with diverting controlled substances. Additionally, the Chiropractic Quality Assurance Commission withdrew charges against Jorge Antonio Barrientos-Lopez, while dental assistant Julie Evette Fernandez faced charges for theft. Health care providers charged with unprofessional conduct have 20 days to respond, after which the case enters the settlement process or proceeds to a hearing.
