The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of several health care providers due to various misconducts. Marty J. Hulm lost his home care aide credential after being placed on the long-term care abuse and neglect registry in July 2024. Certified nursing assistant Henry Mutua Kariuki faced revocation after being substantiated for sexually abusing vulnerable patients between 2022 and 2024. Philip Sulaiman Luseini-Massaquoi also had his home care aide credential revoked for similar reasons and was placed on the abuse registry in April 2025.

Lana Carol Lynn Lortz had her credentials suspended indefinitely for failing to comply with monitoring conditions from a previous stipulation. Additionally, Travis John Starkovich, a certified nursing assistant from Idaho, had his credential suspended following an agreement due to an inappropriate relationship with a client. The Department of Health encourages consumers to report any unprofessional conduct by health care providers. More information about disciplinary actions and provider credentials is available on their website or by calling their customer service.
