The Washington State Department of Health announced disciplinary actions against several health care providers as of January 9, 2026. The department’s Health Systems Quality Assurance Division oversees licensing for over 80 health professions. Complaints against providers include unprofessional conduct, such as inappropriate relationships and failure to comply with regulations.

In Benton County, registered nursing assistant Ramon Castaneda was charged with unprofessional conduct after allegations of inappropriate sexual conduct. In Chelan County, Maria Rosario Arnaga-Rodriguez had her dental assistant application denied due to a felony conviction. King County saw charges against massage therapist Meijun Guo for failing to adhere to previous agreements, while psychologist Steven R. Kubacki received probation.

Other actions included the termination of probation for several professionals and notifications of cease-and-desist orders for unlicensed practice. In Snohomish County, certified nursing assistant Kenneth Karlas Kensinger had his credential reinstated under conditions.

Providers facing charges have 20 days to respond in writing, after which cases may go to a hearing if a settlement cannot be reached. For more information, the public can access the Health Care Provider Lookup on the department’s website or call their customer service.
