Valley Medical Center is conducting a Community Health Needs Assessment (CHNA) to better understand the health needs of the communities it serves. An independent organization, Forward Community Health Consulting, is assisting with this assessment. Community members are invited to participate by sharing their perspectives on significant health issues. The feedback collected will guide Valley in prioritizing health needs and developing strategies to address them. This effort aims to enhance the health and well-being of the community. Community members are encouraged to complete a short survey to provide their input.
