The Washington State Department of Health has announced disciplinary actions against several health care providers. In Clark County, assistant physical therapist Brenda Lee Jensen faces charges of unprofessional conduct for billing for patient care not provided. The department also terminated the probation of counselor Jacob Michael Phillips. In Grays Harbor County, Sarah Sam Sotomish’s application for a home care aide credential was denied due to allegations of financial exploitation and neglect. King County’s Nicci Allen was charged with practicing counseling without a license, while Katlin Marie Kellow was released from monitoring terms of her agreement. Jodi N. Piper in Kitsap County had her home care aide application denied after demanding $10,000 monthly from a client. Samantha Kay Borja from Thurston County was also denied due to placement on a long-term care abuse registry. Rainee Ann Arevalo in Walla Walla County faces charges for inappropriate conduct with incarcerated patients. In a separate case, Julie Evette Fernandez’s dental assistant credential was reactivated under agreed conditions. Following charges, providers have 20 days to respond before the case proceeds to settlement or hearing.
