Every VHA Health Care System now has at least one team recognized as an Age-Friendly Health System, marking a significant achievement in enhancing the well-being of aging Veterans. This initiative, which has been implemented across around 500 VA teams in five years, emphasizes the 4Ms Framework: What Matters, Medication, Mentation, and Mobility. The Age-Friendly Health System aims to better address the needs and preferences of older Veterans.
A success story highlights the impact of this framework through 87-year-old Veteran Manuel Velasco, who faced challenges due to chronic hemiplegia. After being admitted to a Community Living Center (CLC), he expressed a strong desire to return home, feeling trapped and struggling with depression. The health care team prioritized his wish, providing necessary equipment and coordinating home-based care that supported his independence.
Through focused efforts, Velasco was able to return home and live happily with his wife. The initiative underscores a commitment to personalized, holistic care that aligns with the goals of each Veteran, ensuring they receive the dedicated support they deserve.
